چکیده
Pituitary apoplexy is a serious condition, which if left untreated, might lead to irreversible life-long complications. Hence, pituitary should always be included in the differential diagnoses of patient with an isolated sixth cranial nerve (CN) palsy. This report highlights case presenting CN palsy associated apoplexy. Although adenomas are common, they seldom present abducent without any other involvement. The 47-year-old female presented acute right eye pain, diplopia, and squint. Examination revealed unilateral Brain MRI showed sellar suprasellar mass suggestive hemorrhagic was transferred neurosurgery underwent transsphenoidal resection macroadenoma. Postoperative follow-up clinical improvement. It is, thus, imperative for physicians have knowledge recognize its causes.

Isolated sixth cranial nerve palsy as the presenting symptom of a rapidly expanding ACTH positive pituitary adenoma: a case report
BACKGROUND Pituitary adenoma may present with neuro-ophthalmic manifestations and, typically, rapid tumor expansion is the result of apoplexy. Herein, we present the first case of an isolated sixth cranial nerve palsy as initial feature of a rapidly expanding ACTH positive silent tumor without apoplexy. CASE PRESENTATION A 44 year old female with a history of sarcoidosis presented with an iso...
متن کاملThird, Fourth, and Sixth Cranial Nerve Palsies in Pituitary Apoplexy.
BACKGROUND Pituitary apoplexy (PA) often presents with acute headache and neuro-ophthalmic manifestations, including ocular motility dysfunction (OMD) from cranial nerve palsies (CNPs). Our goal was to describe the epidemiology and outcomes of OMD in a large, single-center series of patients with PA. METHODS We conducted a retrospective chart review of all patients with PA seen in our pituita...

Pituitary adenomas may be manifested with visual field defects or rarely in the form of total oculomotor palsy. Visual field defect is most frequently in the form of bitemporal hemianopsia and superior tempo...

Due to its long subarachnoid course, it may be damaged by downward shift of the brainstem as often occurs in increased or decreased intracranial pressure (“false-localizing sixth nerve palsy”). Alternatively, the sixth nerve may be involved in isolation by a compressive lesion in the cavernous sinus or along th...
